Clients are required to submit a Chain of Custody (CoC) form specifying the analyses required, samples descriptions, which provides Fisher with authorization to proceed with analytical work.  Other terms may be negotiated but must be agreed upon before submission of samples.  Click on the link below to download a pdf version of Fisher’s Chain of Custody form.
